Multimodal therapy of a cat with FeLV infection and lymphoma

Zusammenfassung

Ein zweijähriger, FeLV-infizierter Kater wurde wegen reduzierter Futteraufnahme und Schmerzen im Abdomen in der Klinik vorgestellt. Nach bildgebender Diagnostik und Feinnadelaspiration konnte ein intestinales, großzelliges (high-grade) Lymphom mit zusätzlichen Veränderungen in der linken Niere und der Milz diagnostiziert werden. Da bei einem intestinalen Lymphom in Verbindung mit einer FeLV-Infektion gleich zwei Erkrankungen mit schlechter Prognose vorliegen, wurde die Therapie des Lymphoms nach einem modifizierten COP-Protokoll erstmals um den antiretroviralen Integrase-Inhibitor Raltegravir erweitert. Damit konnte bei sehr guter Verträglichkeit der Behandlung eine schnelle Tumorremission beobachtet werden. Der Kater lebt zum Zeitpunkt der Manuskripteinreichung (1125 Tage nach Diagnosestellung) mit gutem Allgemeinbefinden und ohne Tumorrezidiv.

Summary

A two-year-old FeLV infected cat was presented to the animal hospital due to reduced appetite and abdominal pain. Following diagnostic imaging and fine needle aspiration, intestinal lymphoblastic (high grade) lymphoma with additional alterations in the left kidney and spleen was diagnosed. Since the combination of intestinal lymphoma with concurrent FeLV infection comprises two disease entities carrying poor prognosis, treatment with a modified COP chemotherapy was supplemented with the antiretroviral drug Raltegravir. Rapid remission of the tumour together with very good tolerability of the combination therapy was achieved and the cat is alive at the time of manuscript completion (1125 day following diagnosis), in good condition and continued complete remission. To the best of the author’s knowledge, this is the first report of concurrent chemotherapy and anti-viral treatment in a FeLV-infected cat with lymphoma.
