Thoracocentesis – indications, methods, techniques

Zusammenfassung
Die Thorakozentese wird bei Tieren mit einem Pneumothorax oder Pleuraerguss angewendet und ist bei korrekter Durchführung eine sichere Methode, Patienten mit akuter AtemnotLinderung zu schaffen. Typische Indikationen sind der offene und geschlossene Pneumothorax mit traumatischen, spontanen und iatrogenen Ursachen sowie Flüssigkeitsergüsse jeglicher Art (Pyo-, Hämo- und Chylothorax). Die Nadelthorakozentese wird zu diagnostischen und therapeutischen Zwecken angewendet. Muss der Thorax innerhalb von 24–48 Stunden mehrfach punktiert werden oder soll eine Spülung des Thorax aufgrund eines Pyothorax erfolgen, ist das Legen einer Thoraxdrainage zu erwägen. Thoraxdrainagen sind bei adäquater Durchführung mit geringen Komplikationen ohne Probleme und schnell zu legen. Patienten mit Drainagen müssen sorgfältig beobachtet werden, um bei jeglicher Selbstmanipulation eingreifen zu können. Bei versiegender Produktivität über 24–48 Stunden kann die Thoraxdrainage wieder
gezogen werden.
Summary
Thoracocentesis or thoracic drainage is used in animals with pneumothorax or pleural effusions and is, if performed correctly, a safe method to provide relief for patients with acuterespiratory distress. Typical indications are open and closed pneumothorax with traumatic, spontaneous and iatrogenic causes as well as pleural effusions of any kind (pyo-, haemo- and chylothorax). Needle thoracocentesis is used for diagnostic and therapeutic purposes. If the thorax needs to be punctured several times within 24–48 hours or if a thoracic lavage shall be performed, the placement of a thoracic drainage should be considered. Thoracic drainages can be placed easily and fast with minor complications, if performed in a standard correct manner. Patients with drainages need to be observed carefully in order to intervene in any case of self manipulation. The advantages of thoracocentesis and thoracic
drainage clearly outweigh possible complications. The thoracic drainage can be removed, if no more air or fluid can be retrieved for 24–48 hours.
